Four were classified as M&uuml;llerian-type CCCs (MTCCC), arising in the paratestis or seminal vesicle, while three were identified as papillary CCCs of rete testis origin (RTCCC). MTCCCs exhibited features similar to ovarian and endometrial CCCs, with morphological patterns such as glands, tubules, papillae, and solid nests of clear cells, and showed aggressive behavior with frequent metastases, often involving retroperitoneal lymph nodes, lungs, and bones. RTCCCs were confined to the tubular system of the testis (rete testis and seminiferous tubules), showed branching fibrous papillae with clear or eosinophilic cells (yet lacking hobnail cells), and had a more indolent course with no metastases in the cases studied. Immunohistochemical studies demonstrated an overlapping profile in the two subtypes regarding some markers (i.e., positivity for PAX8, AMACR, napsin A, CK7, and wild-type p53 expression) and distinct for others, as MTCCCs are positive for CA125, while RTCCCs lack CA125 expression and are also positive for CAIX and CD10.<\/p>\n<p>&nbsp;<\/p>\n<p>Comment<\/p>\n<p>In the context of genitourinary tumors, CCCs of M&uuml;llerian type have been described as separate entities in the testicular adnexa and the urinary tract, with overlapping clinical features (aggressive course) and immunophenotypic characteristics (positivity for PAX8 and negativity for CAIX). The pathogenesis of these tumors remains speculative, and could be clarified through molecular studies on larger case series. This study, in addition to adding further cases of CCCs to the very few reported in the literature, introduces a new subtype of RTCCC developing within the rete testis, with a more indolent clinical course and an immunohistochemical profile similar to renal neoplasms. This may represent a diagnostic issue: among testicular metastases from solid tumors, those of renal origin are relatively common, and, in one-third of cases, may represent the first clinical manifestation of the disease, according to a recent review [10.3390\/jcm12175636].
