Oncogenic driver mutations were identified in RAS family genes in 34 cases (87%), including HRAS, Q91R, and, less commonly, KRAS and NRAS mutations. The next-generation sequencing of IUP cases also showed a lack of mutations characteristic of urothelial carcinoma, such as FGFR3, TP53, and TERT promoter. Interestingly, statistically significant association with smoking history were found in HRAS-mutated IUPs, whereas KRAS mutations occurred in non-smoking patients with tumors showing cystic changes on histopathological examination. Clinical follow-up showed no recurrence or progression to carcinoma in all IUP cases.<\/p>\n<p>&nbsp;<\/p>\n<p>Comment<\/p>\n<p>Papillary neoplasms of the urinary tract commonly provide diagnostic challenges to pathologists, given their biological variability and the subjectivity of the diagnostic criteria. The relationship between inverted urothelial papilloma and papillary urothelial carcinoma with an inverted pattern was not clear before this timely study. Given the advances in molecular pathology in recent years and the ever-increasing availability of next-generation sequencing, the emergence of such molecular data was only a matter of time. The presence of mutations in RAS family genes proves the neoplastic nature of the IUP, and the lack of FGFR3, TP53, and TERT promoter mutations argues against the relationship with papillary urothelial carcinoma. The molecular findings of this study not only characterize IUP but also potentially enable the use of next-generation sequencing data for the diagnostic purposes of selected challenging cases.
